FREE Sports Physicals for Fayetteville students in current grades 6th - 12th! Next Wednesday, May 6 from 8am - 11am. Students can pick up paperwork in the office (Middle or High School) or at this link https://www.ohsaa.org/medicine/physicalexamform . Parents portion of this form must be completed and returned to the office by Friday.
